After deciding to schedule an appointment, you will be sent two emails from Square Contracts. One will have a credit card authorization form for copays and cancellation fees. The other email will have your Notice of Privacy Practices, Consent Forms, Teletherapy Consent, Insurance Responsibility, and Cancellation Policy to review and sign electronically. These items will be reviewed at your first appointment if there are any questions or concerns.
-
During your first session, we will review the items you signed electronically to clarify any questions that you may have. We will complete two assessments, one for depression and one for anxiety. We will also complete an in depth intake where we explore your history and background, past and current relationships, etc. We will then identify your treatment goals and how we will work towards meeting them.

Weekly sessions are recommended in the beginning of treatment until there appears to be a decline in symptoms that has stabilized, then it is recommended to decrease to bi-weekly sessions. The average time in therapy varies per person. Ongoing progress will be measured every three months at your treatment review.

The cancellation policy requires a 48 hour notice. If you cancel between 48 and 24 hours you will be charged half of the session fee. If you cancel at 25 hours or less you will be charged the full fee of $150. In addition, if you are more than 15 minutes late for a session the session will be counted as a “no-show.”
